微小殘留病灶(Minimal Residual Disease,MRD)檢測,是指通過檢測癌癥患者血液中的循環腫瘤DNA(ctDNA)或骨髓中的微量腫瘤細胞,判斷癌細胞在患者體內的殘留情況,可用于癌癥患者的療效監測、復發風險評估、預后評估和治療干預決策等。
MRD的概念最早在白血病診療中提出,隨著臨床試驗的推進,近年來國內MRD也逐步擴展到實體瘤(肺癌,結直腸癌,乳腺癌,前列腺癌等)的檢測,并逐漸以實驗室自建項目(LDT)的方式參與到臨床決策中。相比MRD在血液瘤中的應用,實體瘤MRD的檢測難度更大:①實體瘤個體差異大,對NGS panel的設計要求更高;②早期腫瘤釋放到外周血的ctDNA含量極低,對MRD檢測靈敏度要求極高。
為了促進實體瘤MRD LDT流程在臨床上的應用和推廣,菁良科技研發了適用于MRD LDT流程性能驗證和日常質控的ctDNA質控品。該產品采用100%細胞系原料制備,通過人工片段化模擬循環腫瘤DNA,產品覆蓋常見腫瘤靶向治療相關基因,共包含37個熱點突變,突變頻率為0%、0.005%、0.05%、0.5%共4個梯度,采用數字PCR進行精確定量,滿足MRD LDT流程的檢測限驗證需求。此外,該產品還通過業內知名的NGS panel——Illumina TruSight Oncology 500 ctDNA panel進行驗證,提供驗證結果供用戶參考。
